This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.

Who we are

M3 Information, LLC operates the M3 Mental Health and Pain Review (m3mentalhealth.org), a screening service. When we verify Medicare eligibility or otherwise act as a healthcare provider, we are required by law to protect the privacy of your protected health information (PHI) and to provide you this Notice of our legal duties and privacy practices.

How we may use and disclose your PHI

Your rights regarding your PHI

You have the right to: inspect and obtain a copy of your record; request correction/amendment; request an accounting of certain disclosures; request restrictions on certain uses; request confidential communications; and receive a paper copy of this Notice. To exercise these rights, contact our Privacy Officer at privacy@m3information.com.

Our duties

We are required to maintain the privacy of your PHI, provide this Notice, follow its terms, and notify you following a breach of unsecured PHI. We reserve the right to change this Notice and to make the revised Notice effective for PHI we already hold; the current Notice is posted at m3mentalhealth.org.

Complaints

You may complain to us at privacy@m3information.com or to the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.
